Vendor Application Form
Please fill out the form below to register as a vendor for Autumn Blaze Festival. Once the form is competed, you will be emailed an invoice with payment options (including online pay via PayPal.)
**Note: All vendor payment MUST be received prior to the event.**

No payments will be accepted on the day of the event and vendors will not be allowed to participate in the event.
Rules and Requirements

Food vendors can set up starting at 8am, and craft vendors can start at 8:30am.

Vendors must be present with their products or food during the entire event (10am – 6pm); no early departures.

Each vendor must adhere to all state and city requirements for local festivals (electrical, liquor, food, etc.)

Exhibitors and vendors must provide their own display, draped tables and/or tents and shelter.

Each tent must have a minimum of 30lb weights on each corner.

All vendors are responsible for reporting sales tax to the proper state authorities.

Spaces are assigned on a first to register basis.

Each vendor is responsible for the cleanup of their space and surrounding area using their own trash receptacles.

Vendors and exhibitors will be sent notification on booth location and directions (you must provide your email address below).

The Autumn Blaze Committee has the right to refuse any and all vendor applications.

No refunds will be issued, under any circumstances.
